Enrollment Information
Parents, school districts, or other interested
parties can write or phone the Motor Development Clinic for an application
form. If an opening is unavailable, the child's name is added to the
current waiting list. When an opening does occur each client becomes
involved in the following process: 1) assessment; 2) consultation meeting
with parent and clinic staff; 3) a written ITP developed by the clinician,
clinic coordinator and clinic director; and 4) initiation of the movement
therapy program.
The ITP contains program recommendations,
goals, and objectives which aid the clinician to develop appropriate
lessons. All activities are approved by the clinic coordinator.
All assessments, ITP's and other materials
developed for the client are given to the parents. Upon request, and
when appropriate, copies of this material can be sent to other agencies
working with the client.

Please note that as of Summer
2007, the Cal Poly Motor Development Clinic will no longer accept
fees paid directly by the local Regional Centers.
If you are affiliated with one of the Regional
Centers, it will be up to each individual parent to pay the program
fee directly to the Motor Development Clinic. Any
type of reimbursement will be between you and your Regional Center.
We will not be involved with the collection of your fees.
